Effective 11/01/2025, family membership plans (involving parents + minor children under 18 years of age) will be phased out of the practice.

New Pricing will be as follows for all patients as individual plans:
65 y/o and above:Â $129/month.
18 to 64 y/o:Â $109/month.
Under 18 y/o:Â $89/month.

Re-enrollment fee (60+ days past last payment, subject to approval)

Billing & Due Dates
Membership fees are due on the 25th of each month
Memberships that start after the 15th of the month are prorated
Cash, Check, Credit/Debit Card, and ACH (automatic bank draws) accepted.
Membership service prices are subject to change (45 day written notice to current members)
Late fee applies if 3 days past due
