Health Care Journey

Please allocate 90 to 120 minutes for your first office visit in order to do a complete review of your past medical/surgical/family/social history, medications, and assess your current symptoms with Pre-Visit Questionnaires that are completed 24 to 48 hours PRIOR to your first visit.

After a thorough physical examination and discussion, you will have a written Plan of Care to take with you to start your Healthcare Journey.

You may pay with cash, check, debit/credit cards, ACH/ Electronic Bank Transfer, which can be automated by the practice. Patients may elect to pay monthly or pre-pay for 3, 6, or 12 months.

Health insurance cannot be used to pay for DPC memberships/services, but some employers allow HSA funds to cover membership fees/services.

For non-urgent requests, patients may utilize their free Patient Portal to send secure messages [1-3 day response time].

For time sensitive/same day requests, patients text the doctor directly during normal business hours [1 to 4 hour response time].

For urgent requests, patients can text/call doctor [30 to 60 minute response time].
